With HORIZON®, POLARIS Laboratories®' online reporting software, you can virtually eliminate filling out Component Registration Forms and sample jar labels, even if it's the first time a unit is being sampled. Learn More.
Sample Jar Label & Component Registration
A sample jar label and component registration form are provided for each test kit ordered.
Fill out the sample jar label completely and accurately.
Include all unit and fluid information requested including unit ID, type of component and its position, time on both the fluid and the unit and designate whether or not the fluid has been changed since the previous sample was taken.
Affix the completed jar label to the white sample jar.
Fill out the component registration form only when sampling a component for the first time or to notify the laboratory of a change in component and/or fluid information already registered with the laboratory. Place the completed registration form, along with the labeled sample jar inside the black mailer.
Affix the return address label appropriate to the laboratory location nearest you to the black mailer and use a trackable mail service for shipping it to the laboratory. Ship all samples the day they are taken.

You can eliminate using the component registration form by printing and completing an equipment list, filling in information for each unit you intend to test and faxing it back to the laboratory for a one-time import before you begin sending in samples.
